1. Read through the Lindsay's Law document, and watch the required video.

1. Legible copy of the player's Birth Certificate.
2. Copy of the player(s) report card showing what grade they will be in for the 2021-2022 school year.  If the player(s) is just entering Kindergarten, a letter of enrollment will suffice.
3. Confirmation of Residence.  Copy of any bill or utility bill that shows your address.  Be sure to cover any information that does not need to be seen.  If you have any questions, your head coach or team mom can assist.
4. Sign the document below, the LMYFC Parent/Guardian Code of Conduct.
