How it works:  The LMYFC Board will review each request on an individual basis, after reviewing any and all documents that are attached, reviewing your level of volunteer support in the past, and consulting with your head coach from last season, we will communicate on the status of your application either via e-mail or by telephone.

Please note that this form is only visible by the LMYFC Board and is not viewable to the public in any way.  We, as the LMYFC Board, respect each individual's privacy and will not disseminate any information that is submitted through this form.
